Which celebrity died in a totally silly way that could have easily been avoided?

 

Actor Jack Cassidy. At about 4am on December 12, 1976, he sat down on the couch in his penthouse apartment. He poured himself a scotch and soda and lit a cigarette. About 2 hours later, fire fighters found his charred remains in the front hall. It was determined that he fell asleep, and when he woke up, the place was on fire. He tried to crawl to the front door, but died from lack of oxygen at the age of 49.
